Improved SQL and version-control correctness, crash recovery, and concurrent-write protection.

## Reliability and correctness

- Prevented stale transactions and failed version-control operations from overwriting concurrent writes.
- Fixed recovery after interrupted writes following `dolt_gc()` or `VACUUM`.
- Corrected schema merges, conflict aborts, and workspace constraint enforcement.
- Fixed historical queries, blame, diffs, and SQL patches involving primary-key affinity and collation.
- Improved checkout, clone defaults, remote argument handling, and WAL transitions for SQLite files.

## Performance and testing

- Accelerated secondary-index row fetches.
- Expanded concurrency, recovery, and Dolt oracle coverage; strengthened nightly performance regression checks.
